The Sanitarium is unlocked by reaching Week 4 and has three places that can be upgraded:
- Treatment Library: reduces the cost of Sanitarium Quirk treatments, allowing you to lock in positive Quirks and remove negative ones for less gold.
- Medical Devices: reduces the cost of treating Disease and increases the chance of wiping out more than one disease at a time.
- Patient Cells: this allows for more heroes to be treated at once. Initially, the Sanitarium only has one medical slot and one treatment slot.
Quirk Management[]
When committing a hero to a quirk treatment slot, the player can remove up to 1 negative quirk and lock/remove up to 1 positive quirk. When managing a positive and negative quirk at the same time, the player pays for the sum of both individual treatments. This can make quirk treatment very expensive, and using the Sanitarium for this purpose is often put off until much later in the game where a player can afford regular treatments. On the other hand, triggering Caregiver's Convention by completing a Gather quest in the Weald will make the Sanitarium completely free for the next week, saving tens of thousands of gold on treatment.
Removal of chronic negative quirks is a more costly undertaking than treating a new negative quirk, so it's often recommended to get problematic negative quirks treated as soon as possible. If the player cannot afford treatment, it's recommended to not send the hero with that quirk on adventures to avoid the risk of the negative quirk becoming chronic. Sometimes it's not imperative that all negative quirks be treated - a chronic but benign negative quirk (for instance, one that restricts the stress relief activities a hero can participate in) will never be replaced by new negative quirks, leaving less room for other more problematic quirks (like Kleptomania or stat-reducing quirks).
Up to three positive quirks per hero can be locked, preventing new quirks from replacing them. However, should a player find a new quirk they want to lock while they already have three quirks locked on a hero, the hero must spend two weeks at the Sanitarium - one to remove a locked quirk, and another to lock in the desired quirk. It is also possible to remove non-locked positive quirks; the reason for doing so is mainly to delete redundant/weaker positive quirks so that new positive quirks are less likely to overwrite more important ones.
Diseases were once very impactful negative quirks, but now take their own slot, can be resisted, and are much cheaper to treat than quirks. Disease treatment is different from quirk treatment, and as such a hero committed to disease treatment cannot have their quirks treated in the same week.

The cost of treating heroes increases depending on their resolve level. For reference, Apprentice Heroes are Level 0-2, Veteran 3-4, Champion 5-6.

Spreading the Curse[]
If you have the Crimson Court DLC added, and you put a hero afflicted with the Crimson Curse in the Sanitarium, they could infect another hero being treated there. If you've defeated the Countess and can treat the Crimson Curse like any other disease, bear in mind that the Crimson Curse can still spread this way, even to other heroes under regular disease treatment.
